Caring Partners of Morris & Sussex is a non-profit care management organization that works in partnership with the primary organizations that serve children and families in Morris and Sussex Counties. The primary goal of Caring Partners is to maintain children with severe emotional and behavioral challenges, developmental disabilities and/or substance abuse issues safely at home, in school and in the community by providing support, education, and advocacy and by connecting children and families to the needed services and resources. Caring Partners believes in working with families in a strength-based manner and believes that children are best served in their homes or communities using natural supports.
The Care Manager develops partnerships with families and coordinates their care with other system partners. He/she facilitates team process used to build on family and community strengths in order to address family needs. The Care Manager Develops and implements crisis plans. This position requires a bachelor’s degree in a related field and two years of related experience. It also, requires a valid driver's license and clean driving record. Bi-lingual and computer literacy are assets. Experience working with individuals with developmental disabilities is also an asset. Candidates must have excellent communication, organizational and assessment skills.
