View More FAQsWe are a non-profit care management organization that works in partnership with the primary organizations that serve children and families in Morris and Sussex Counties. Our mission is to help and maintain children with severe emotional and behavioral challenges, developmental disabilities and/or substance abuse issues safely at home, in school and within the community. We do this by providing support, education, and advocacy to children and families and connecting them with the needed services and resources. Caring Partners believes in working with families in a strength-based manner, and believes that children are best served in their homes or communities using natural supports.
Wraparound is a planning process that helps children, adolescents, young adults and their families realize their full potential. Wraparound uses people from the child’s and family’s life to create a Child and Family Team (CFT). These team members work together to help create an Individualized Service Plan (ISP) to address their needs in areas including, but not limited to: family, safety, emotional/psychological supports, education, legal issues, spiritual life, and medical concerns. We “wrap” services around children and their families to help them remain at home, in school and in their community.
A Child & Family Team is comprised of the people who are integrally involved with the family and play an important role in the child’s life. Team members may include relatives, clergy, friends, neighbors, or coaches, as well as more "formal supports" like therapists, teachers, probation officers or other professionals who provide services to the family.
The child’s primary caregiver is a key member of the Team and must take ownership of the family's Individual Support Plan (ISP) in order for it to succeed. The Plan addresses all areas of the child’s life, including family interactions, safety concerns, emotional/psychological supports, educational objectives, legal issues, and medical concerns.
Team members are required to commit to doing “whatever it takes” to help the child and the family achieve positive outcomes. Family preferences, culture, and values will determine the size and composition of the Team, which also evolves and changes over time as the family relationships, interests, and needs change.
The Child & Family Team’s ultimate objective is to use wraparound values and strength-based planning to continually develop, refine, and successfully execute the family's Plan to a successful outcome.
The success of care management is built through collaboration with a rich array of qualified healthcare and service providers.
